WAIVER OF LIABILITY ON BEHALF OF ADDITIONAL GUESTS: On behalf of the guests that I register to attend, I am signing the below “Waiver of Liability Relating to COVID-19”, “Waiver of Lawsuit/Liability”, and “Photo/Video Release.”

WAIVER OF LIABILITY RELATING TO COVID-19: The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is reported to be extremely contagious. Camp Odayin and/or venue cannot prevent you from becoming exposed to, contracting, or spreading COVID-19 while participating in the Have a Heart Benefit event. It is not possible to prevent against the presence of the disease. Therefore, if you choose to participate in the fundraising event you may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19.

WAIVER OF LAWSUIT/LIABILITY: I hereby forever release and waive my right to bring suit against Camp Odayin, the venue, and its owners, officers, directors, managers, officials, trustees, agents, employees, sponsors, and/or other representatives in connection with the risks assumed by participating in the fundraising event. I understand that this waiver means I give up my right to bring any claims including for personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.

PHOTO/VIDEO RELEASE: I hereby grant full permission to any and all of the above parties to use any photographs, video recordings, website images, audio recordings or any other record of this event.
