Thank you for registering for the Whole Heart Golf and Pickleball Tournament on June 10, 2024. We ask that each golfer and pickleball player complete their own registration.

GOLF TOURNAMENT AND DINNER
11:45 am shotgun start
4-person scramble format with fun field events, contests, and prizes
$190 per golfer includes lunch, golf, cart, driving range, and dinner

PICKLEBALL TOURNAMENT AND DINNER
Two tournaments - for both social and competitive doubles teams

2-3:30 pm social play - round robin and tournament (beginners up to 3.5 rating)
3:30-5:00 pm intermediate play - round robin and tournament (3.5+ rating)

5:00 pm happy hour with live music on the patio and silent auction
6:00 pm dinner and program
$85 per person

DINNER & LIVE MUSIC ONLY

Waiver

WAIVER OF LIABILITY RELATING TO ACCIDENT/INJURY: I understand that participating in Camp Odayin’s golf tournament and pickleball tournament at Dellwood Country Club is potentially hazardous, and that I should not enter and participate unless I am medically able and properly trained. In consideration of the acceptance of this entry, I assume full and complete responsibility for any injury or accident which may occur while I am traveling to and from the event, during the event, or while I am on the premises of Dellwood Country Club. I am also aware of and assume all risks associated with participating in this event, including but not limited to, falls, contact with other participants, effect of weather, traffic, and conditions of the road, court or golf course.

WAIVER OF LAWSUIT/LIABILITY: I hereby forever release and waive my right to bring suit against Camp Odayin, Dellwood Country Club, and its owners, officers, directors, managers, officials, trustees, agents, employees, tournament sponsors, and/or other representatives in connection with the risks assumed by participating in the golf tournament or pickleball tournament. I understand that this waiver means I give up my right to bring any claims including for personal injuries, death, disease, COVID-19 or property losses, or any other loss, including but not limited to claims of negligence and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.

PHOTO/VIDEO RELEASE: I hereby grant full permission to any and all of the above parties to use any photographs, video recordings, website images, audio recordings or any other record of this event.
